The transition planning process should enable students to move successfully from school to post-secondary education and training, employment, independent living, and community participation based on the student's preferences, interests, and abilities.

 

Person centered planning has emerged as one mechanism for young people to build self-determined futures with the help of a supportive group of people.

One-on-One support is offered to families of youth who are currently on an IEP

(Individual Education Plan) and are

transitioning from high school into adulthood.
